“What do I do With My Children?”

 

        During various times of the school year, the Deerfield School District will not be open for school. Many of you are wondering what to do with your children while you are at work. To help your family out the Deerfield Community Center will be holding a mini School’s Out Club. This program will provide a safe, structured and supervised environment for children in Kindergarten through 6th grade. If you have any further questions regarding the program contact DCC Staff at 764-5935.

 

                When:  The club will meet on the following dates:

                                       Sept. 21, Oct. 25 & 26, Nov. 21, Jan. 25, Feb. 22, and April 11.

 

                Times:   On days that there is no school the club will be open

                                         from 8:00-3:00pm.

 

                                         When there is an 11:00am early dismissal camp will run from

                                         11:00-3:00pm. Center staff will meet students at the doors

                                             near the playgrounds and escort them back to the center.

 

                Where:  Deerfield Community Center

 

                Grades:  Kindergarten – 6th Grade

 

                Cost:  $12.00 per child per day - Maximum $24.00 per family per day

 

                Meals:  Children will need to have breakfast before attending the

                                        program, each child will need to bring their own sack lunch,

                                        the center will provide an afternoon snack.

 

                Registration: Please fill out the registration form on the back and

                                                   return it to the Deerfield Community Center by

                                                  Thursday, September 13, 2007.

Discipline Policy: All students attending the School’s Out Club must act in an appropriate manor; follow the direction of the adult leader and student volunteers. The following are the consequences for breaking the School’s Out Club rules and the DCC rules:

 

                        1st offense:      Student(s) will be reminded of the rules when they are broken.

                        2nd offense:     Student(s) will be given a time-out. The time will depend on the

                                                severity of the crime.

                        3rd offense:      Parents will be notified of the child’s behavior and appropriate

                                                action will be discussed.

I, the parent/guardian of the registrant, a minor, agree that the registrant and I will abide by the rules of the Deerfield Community Center (the "DCC"), its affiliated organizations and sponsors. Recognizing the possibility of physical injury associated with youth programs (the "Programs") and in consideration for the DCC accepting the registrant for its Programs and activities, I hereby release, discharge and/or otherwise indemnify the DCC, its affiliated organizations and sponsors, their employees and associated personnel, including the owners of fields and facilities utilized for the Programs, against any claim by or on behalf of the registrant as a result of the registrant’s participation in the programs and/or being transported to or from the same, which transportation I hereby authorize.  As the parent or legal guardian of the above-named player, I hereby give consent for emergency medical care given under whatever conditions are necessary to preserve the life, limb or well being of my dependent.
