“Spring Break Day Camp 2008”
This program will provide a safe, structured and supervised environment for children in K-6 grade. Below you will find information regarding the program, if you have further questions contact Julie at 764-5935.
When: Monday, March 24 – Friday, March 28
Times: 8:00-4:00pm (There will NOT be drop in this week)
Where: Deerfield Community Center
Grades: Kindergarten – 6th Grade
Cost: $16.00 per child per day
(Maximum $32.00 per family per day)
Meals: Children will need to have breakfast before the program, each child will need to bring a sack lunch, and the center will provide an afternoon snack.

Discipline Policy
All students attending the Spring Break Day Camp must act in an appropriate manor; follow the direction of the adult leader and student volunteers. The following are the consequences for breaking the Spring Break Day Camp rules and the DCC rules:
1st offense: Student(s) will be reminded of the rules when they are broken.
2nd offense: Student(s) will be given a time-out. The length of time will depend on the
severity of the rule that was broken.
3rd offense: Parents will be notified of the child’s behavior and appropriate
action will be discussed.

IMPORTANT Waiver of Participation - Please read and sign the following I, the parent/guardian of the registrant, a minor, agree that the registrant and I will abide by the rules of the Deerfield Community Center (the "DCC"), its affiliated organizations and sponsors. Recognizing the possibility of physical injury associated with youth programs (the "Programs") and in consideration for the DCC accepting the registrant for its Programs and activities, I hereby release, discharge and/or otherwise indemnify the DCC, its affiliated organizations and sponsors, their employees and associated personnel, including the owners of fields and facilities utilized for the Programs, against any claim by or on behalf of the registrant as a result of the registrant’s participation in the programs and/or being transported to or from the same, which transportation I hereby authorize. As the parent or legal guardian of the above-named player, I hereby give consent for emergency medical care given under whatever conditions are necessary to preserve the life, limb or well being of my dependent.
